need recommendations for best practices/software on meeting notes

The startup where i work has not done a good job of keeping good (any) meeting notes. This is going to be a problem if people challenge us later on about sales forecasts or retained earnings.

What is some good software/technology for a person to keep track of corporate meeting notes (and don’t tell me Microsoft word).

ideally, the system would have a way to show topics, outcomes, action steps, and the teammate’s name that the action step is tied to.

I’d personally use One Note and design the workbook for what you need. But seems like you need something that can’t be edited after the fact. . . pen and paper?
